Thoracic Radiculopathy

Symptoms

Pain, burning, tingling, or numbness may wrap from the mid-back around one side of the rib cage. Symptoms can mimic cardiac, pulmonary, gastrointestinal, or skin conditions and deserve careful evaluation.

Diagnosis

The history and sensory examination help localize a nerve level. MRI can identify a disc or foraminal cause after important non-spinal conditions have been considered; electrodiagnostic testing is occasionally useful.

Treatment options

Initial care may include medication, activity modification, therapy, and a targeted injection when the diagnosis is well supported. Treatment is tailored to the cause rather than the pain pattern alone.

When surgery is considered

Surgery is uncommon but may be considered for persistent disabling symptoms, progressive weakness, or a focal compressive lesion that clearly matches the clinical findings.

When to seek care promptly

Because symptoms can mimic heart, lung, or abdominal conditions, new band-like chest pain should be evaluated — and spinal causes considered once other causes are addressed.
