ALIF

Anterior lumbar interbody fusion — reaches the lumbar disc space from the front of the body to restore disc height and fuse selected levels.

What it is

ALIF approaches the lumbar disc space from the front of the body. Through an abdominal incision — made with assistance from an access surgeon — Dr. Choi removes the damaged disc and places a larger interbody implant that restores disc height and supports fusion of the treated level.

The anterior route allows direct access to the front of the disc space and accommodates implants that can restore height and alignment. Additional posterior fixation may or may not be needed, depending on the case.

When it may be considered

ALIF may be considered for specific patterns of disc collapse, instability, deformity, or spondylolisthesis, and in some cases after prior surgery. Access anatomy — including the position of blood vessels and prior abdominal procedures — and the need for additional posterior fixation are individualized.

What to discuss at consultation

  • Goals: restore disc height and alignment, stabilize the level, and address the structural source of pain or nerve symptoms.
  • Alternatives: nonoperative care, or other fusion approaches such as LLIF or TLIF / PLIF, chosen according to anatomy and goals.
  • Recovery: a hospital stay followed by activity restrictions and a gradual return to function while the fusion matures.
  • Risks: access-related considerations unique to the anterior approach, fusion, and the individual risks of surgery — reviewed in detail before any decision.
